STEP BY STEP

The fifty minutes, broken down

0–15

History, properly taken

What you were doing when it started, what makes it worse, how you sleep, what you have already tried. Most of the diagnosis is in here.

15–30

Physical examination

Movement testing, joint assessment, orthopaedic tests and neurological screening where the symptoms warrant it. Baseline measurements are written down so visit four has something to compare against.

30–35

What we think it is

Explained in plain language, including what we are not sure about. If it is not a chiropractic case you find out here, and the visit ends with a referral.

35–48

Treatment, if appropriate

Manual therapy, mobilisation, soft tissue work or adjustment — with the technique explained before it happens and an alternative offered if you would rather.

48–50

The written plan

Expected number of visits, what improvement should look like, what you do at home, and the date of your four-visit review. You leave with a copy.

On risk, honestly

Spinal manipulation carries a small risk. The common one is soreness for a day or two after treatment, which happens to roughly a third of people and settles on its own. Serious complications are rare but they are not zero, and the evidence on neck manipulation in particular is debated within the profession.

You will be told what technique is planned and why, and mobilisation or soft tissue work is always available instead of a high-velocity adjustment if you would prefer it. Saying no to a technique does not change your fee or your plan.
